Services: CHILD HEALTH : Provides well child physical, developmental screening, vision and hearing screening, community referrals, nutrition education, and social assessments.
ENERGY ASSISTANCE: Assists low income home owners and renters pay for a portion of their primary heating costs for the winter heating season Assistance is based on household income, size, type of fuel and type or housing The application period is from November 1 through April 15th: must meet 150% of poverty level
FADSS: FADSS is an in home family development program for FIP recipients and their families which utilizes a strength based approach to improve family function and move families toward self sufficiency: must be a current FIP recipient, referred by DHS, Promise Jobs or self referral
Head Start: Provides comprehensive child development experience to children ages three to five Comprehensive social services, health, education, nutrition, and parent involvement components are also provided Volunteer role model/mentors are needed at breakfast and lunch times: must meet income guidelines and bring a birth certificate, immunization records, income statements and Social Security card/number
Maternal Health: Prenatal education services: must be at or below185% poverty level
WEATHERIZATION: Provides major energy efficiency services without cost to homeowners/renters who meet guidelines Work includes sealing air filtration leaks, high-density insulation, health and safety check, heating system repair or replacement, refrigerator diagnostic and replacement along with utility measures such as aerators, low flow showerheads, pipe wrap, waterbed blankets, carbon monoxide detectors, and compact florescent light bulbs Some minor repair is allowable for health and safety reason: must be at or below the 150% poverty level.
WIC: Provides a health screening, nutrition education, breast feeding promotion/support along with a supplemental food package for pregnant, postpartum and breast feeding women, infants and children birth to 5 years old Provides immunizations, lead screenings and care coordination for medical and dental services in are for children birth to 21 years old: must be below 185% poverty level.
