Provides case management and assistance to homeless veterans who are in need of health care, mental health services, and housing services. Administers the HCHV, Health Care for homeless Veterans, and the HUD-VASH, VA Supportive Housing programs. HUD-VASH provides intensive case management combined with independent supportive living services through Section 8 Housing Choice Vouchers. A preliminary application must be filled out for HUD-VASH, along with a referral from VA staff.

The Grant Per-Diem (transitional housing) program has 188 beds available at different sites in the community.

The Veterans Justice Outreach program provides resource linkages to veterans in jail or prison and transitioning back into the community in danger of being homeless.
