Services: Lifeline reduces the monthly telephone bill by up to $9.25 for low-income customers. To Apply: Contact Frontier and place an application for service. A form will be mailed to you; complete the form, sign it, and return it to Frontier at the address specified on the form. The discounts will be applied when Frontier receives the completed form showing you are eligible for these programs Assistance with connection charges is also available for qualified residents of Federally Recognized Tribal Lands.

Eligibility: Recipients of SSI (except Medicare, Social Security Disability, and Social Security benefits), Food Stamps, Work First, TANF, Medicaid, low income home energy assistance, and Section 8, and the Free National School Lunch program; and customers with income at or below 150% of the Federal Poverty Guidelines.
