Eligibility: Once every Six months with a referral. 2-1-1 may only refer one household per day, with written referral to agency.
Documents To Bring: Picture ID, A hand signed referral from a Social Worker or Case Manager. Referral must include: names and ages of all family members (use space on back to list additional household members under 18), and birth dates for all adults. Anyone over the age of 18 in the household needs a separate referral. If two adults in the same household are not married, they must have two separate referrals. Adult children (over 18) living in the household, must have a separate referral.
