If you do not have a lease agreement you must provide a written statement from your landlord that includes monthly rent amount, the total amount owed, and his/her contact information. G) All recent mortgage documents including payment vouchers. H) If applicable any eviction or foreclosure notices.

Utility Service Must Be In The Name Of The Person Requesting Assistance Except In The Case Of A Widow/widower. No Amounts Can Be "held-over" By The Utility Department Under This Program. Cannot Pay Utility Deposits And Cannot Assist If The Utility Service Has Been Disconnected. Budgeting Class Completion Will Be Required For Further Utility Assistance After The First Time. Cannot Assist With Old Bills At Previous Address. (note: When Efsp Funds Are Available, Utility Assistance Is Granted In The Amount Of One Months Arrears. In These Cases, The Regular Bill Showing The Amount Of Arrears Will Be Required.rent/mortgage: Assistance Given One Time In A Twelve Month Period With Maximum Assistance Of One Months Rent/mortgage Payment Up To $400.
