Services: Lifescape's SeniorAssist program provides information and assistance to persons age 60 or older to access needed services or benefits to retain or attain self-sufficiency or independence. These services may include: energy assistance, pharmaceutical assistance, housing assistance, resolution of issues with Medicare or social security, apply for Medicare D or resolve issues with Medicare D plan, find assistance for home repairs or other needs that threaten the person's independence, referrals for services such as homemakers, home health aid, counseling, etc. Follow-up is part of service.
Eligibility: Age 60 or older. Eligibility requirements for some benefits are determined by the particular benefit that SeniorAssist may help participants to access.
