Services: A resource and referral center that connects individuals to programs within the Conference or refers out to community resources. Also helps qualifying low-income individuals and families with rental assistance, goal setting and budgeting. Provides bus passes for interviews or for first week of employment if job is confirmed. Toiletries/personal grooming items provided as available. Clearinghouse Program: Provides rental assistance and other housing-related needs to mentally ill homeless, transitioning youth/adults (age 18-23), residents of a community rehabilitation residence and individuals being release from incarceration. Homeless Supportive Services: provides intensive case management and referrals to other support services for single, adult, chronically homeless individuals. Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. Pharmaceutical Assistance Program: Provides uninsured Lehigh County residents with once-a-year emergency pharmaceutical assistance of up to $50. Must meet income guidelines. Assistance for medications for asthma, epileptic seizures, diabetes, high blood pressure and antibiotics only. Social Security Representative payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.
