Current Year Documents neededto apply: (1) social security cards for all household members, (2) proof of income for all household members, (3) proof of participation in one of the following programs: food stamps, SSI, Disability, LIHEAP,retirement income, proof of residence in public housing, or participation in the Families First Program; must bring letter showing participation in program with case numbers; OR completion of signed, self-declaration income statement (this must be written by someone that does not live in the household with you who can verify information along with a telephone number; must be dated.
