You must have been diagnosed with Multiple Sclerosis or one of the related MS type diseases which are Clinically isolated syndrome, e.g. optic neuritis; Diffuse cerebral sclerosis (including Schilderís Disease); Acute disseminated encephalomyelitis (post-infectious Encephalomyelitis or ADEM); Baloís disease; Neuromyelitis optica (Devicís disease or Devicís syndrome); Transverse myelitis.
And you must not have received financial assistance from us in the past two years from this request date.
