Services: The Homeless Prevention and Rapid Re-housing Program (HPRP) is a collaboration between Bringing Our Community Home, Casa Esperanza, Catholic Charities, Legal Aid Foundation, Transition House, and the Rental Housing Mediation Task Force.The services offered are financial assistance for rent due, deposits, utilities, moving costs, help with locating housing, legal aid to inform people of their rights, and mediation to help prevent evictions.Santa Barbara County 2010 Very-Low Income Limits: (Household Size/Maximum Annual Income)
Eligibility: The services are available to people who have an income of 50% or less than the Area Median Income, (see chart in program description), and are at risk of becoming homeless, or are currently homeless, and have an eviction notice with their name on it. Services: Provides services to people affected by kidney disease and offers the public information and education on the topic. SERVICES: 1) PATIENT SERVICES: Offers assistance for patients and their families who are waiting for a transplant, have just received a transplant, or are receiving kidney dialysis. Provides medical alert identification jewelry upon requestthrough kidney patient's social worker at the patient's treatment facility.Also maintains an information library containingpamphlets and videos on kidney disease research, treatments and nutrition.2) FINANCIAL ASSISTANCE: Provides assistance for food, medicine, rent and treatment related transportation. Also offers emergency transportation fund for round trip transportation to and from dialysis in emergency situations only.3) COMMUNITY AND PROFESSIONAL EDUCATION: Provides free information on kidney disease and free health screenings for the community.Gives seminars on current developments in kidney research and treatment for health professionals at a nominal cost.Also funds local renal research andeducates the public on the need for organ donors. Call to have an organ donor card mailed to you. 4) DONATIONS: Accepts used vehicles registered with the Motor Vehicle Administration.

Services: This program is for city of Ventura residents only. This is offered as a one time request per year for any household applying for this assistance, these limited funds are available to local residents who would be homeless but for this assistance.

Eligibility: Current resident of the city of Ventura. Currently have housing in the city. Only one member of the household may apply for funds. Funds may be used one time in a 12-month period. Agree to allow information to be shared with other VHPF provider organizations in order to prevent a double allocation an to insure the recipient is able to maintain housing once they,ôve received these funds. Case manager must confirm an extraordinary circumstance. They must confirm that these funds are the right funds to access and that the individual is not likely to be right back in the same circumstance within the year. Only the individual who is at risk of homelessness may request the funds.(The landlord or property management or mortgage company cannot request funds; however checks must be made out to those entities.)
